This is all made possible by the clinicians and staff in the clinics who want to see success for their patients.

Dr. Jade Medeiros PT, DPT has served as the Clinic Director in Middletown for almost two years. When she joined Performance in 2020, she was excited to treat her local community for the first time in her career, and considers it a privilege to treat friends, family, and fellow islanders.  She loves that she can go to an exercise class, walk on the beach, or a restaurant downtown and see her patients and the progress they’ve made.

Dr. Michelle Fuleky PT, DPT, OCS serves as the Clinic Director for Performance’s newest clinic in the South County Commons. Since the clinic’s opening in 2021, she loves being able to provide more access to patient care in such an accessible, community-centered area of South Kingstown. She is excited to help her patients in southern RI return to the activities they love to do as well as empower them to try something new!

Dr. Leanne Monteiro PT, DPT, OCS has served as the Clinic Director in Richmond for three years.

Her goal is to have every patient who walks into the clinic feel empowered to live without pain or limitation. She loves that she can mirror the close-knit community spirit of Richmond by providing a supportive environment for her team and helping her patients get back to what they love, whether that be work, sports, or leisure.
